Three differences between TOEFL and Cambridge First or Advanced:
1.
TOEFL is more for news readers; the Cambridge exams tend more towards literature.
If you read newspapers, prepare for the TOEFL.
If you read novels, try for one of the higher Cambridge exams.
If you don’t read at all, you are probably in trouble.

The best way to prepare for the TOEFL or for any other high level English exam is to read regularly and slowly, preferably a great newspaper like the IHT = International Herald Tribune , a gold mine for all the relevant vocabulary.
In most places the exam is taken online. The fee is around US$ 225.
