Consider that a form is more tiring on the reader and much less inviting than a letter.
On each of the papers that you send you should add
– your complete name
– your address
– your phone number
– the reference of the ad you are answering : a name, a number, or a department.

To put together a complete job application — writing, getting photos, photocopies, and references — could take you several hours, but only the first time! So try to hold out and do it an hour a day for almost a week, until you got it all figured out.
After that, it will be easy and fast to make copies for different applications and for some of your friends, too :-)
